01 / UNDERSTANDING
Learning objectives
Cover inherent exposure, control design, operating effectiveness and residual risk. Use one payment process to connect a specific failure with a specific control and evidence. The objective is to assess what remains vulnerable and justify the conclusion, rather than assign a reassuring rating because the process has a documented procedure.
02 / UNDERSTANDING
The concept in context
An operational risk can arise from people, processes, systems or external events affecting an activity. Assessing inherent risk considers the exposure before the relevant controls, while residual risk considers what remains after their effect. The definitions and rating method should be clear so different teams evaluate comparable situations consistently.
03 / UNDERSTANDING
A worked learning case
A fictional payment process requires independent checking before release. The procedure appears sound, but sampled records show the second approval regularly occurs after execution. The assessor separates design from operation, records the remaining exposure and assigns corrective action. The absence of a recent loss does not erase the observed failure in the control's timing.
ORIGINAL EDUCATIONAL PRACTICE
Try it before you check.
Does a well-written control procedure prove that the control is operating effectively?
Show the answer and reasoning
No; performance needs evidence from the actual process.
A procedure can describe an appropriate check while staff perform it late, inconsistently or without the required independence. Operating evidence tests whether the intended protection occurs at the right point. Assessment should connect that evidence with the identified failure and residual exposure, instead of treating the existence of documentation as a complete assurance result.
Two traps to watch for.
- Rating a control effective solely because a policy exists.
- Using absence of loss as proof that a vulnerability is harmless.
YOUR TOPIC-SPECIFIC LEARNING MAP
Your revision map
State the failure the control is intended to prevent or detect.
Separate design evidence from evidence of actual operation.
Assign corrective action and a check that verifies its completion.
Explain the idea, apply it to a new situation, then check what changed in your reasoning.
