Study plan: Operational risk and loss prevention

Identify failures involving people, processes, systems or external events, then select controls that address the cause rather than merely restating the loss a branch wishes to avoid.

Plan your preparation

Use one branch-process case per revision session. Alternate routine entry errors and disruptive external events so operational risk does not become a synonym for employee carelessness.

02 / UNDERSTANDING

The concept in context

Operational risk concerns losses arising from failed or inadequate processes, people, systems or external events. A wrong beneficiary entry, system outage or internal fraud can create different operational problems. Identifying the event is only a start; the underlying cause and the affected process determine which control might be useful.

03 / UNDERSTANDING

A worked learning case

Original educational case. A clerk selects the wrong beneficiary from two similar names. A second employee approves without comparing the account details, so the transfer proceeds incorrectly. Adding another signature alone may not help. A better analysis asks what independent verification should occur, how similar records are displayed and how errors are detected and escalated when they happen.

ORIGINAL EDUCATIONAL PRACTICE

Try it before you check.

Original practice: An independent account-detail check before release is primarily preventive or corrective?

Show the answer and reasoning

Preventive.

The check acts before the erroneous payment is released, aiming to stop the failure from occurring. A recall attempt after release would be corrective, while a later exception report would be detective. Classifying a control by when and how it works helps assess whether the process has meaningful coverage at each stage.

Two traps to watch for.

  • Calling an extra approval effective without defining what is checked.
  • Ranking risk by past losses while ignoring severe near misses.

YOUR TOPIC-SPECIFIC LEARNING MAP

Build your next study session

  1. Separate event, cause, consequence and control.

  2. Classify controls as preventive, detective or corrective.

  3. Describe one weakness that remains after each control.

Explain the idea, apply it to a new situation, then check what changed in your reasoning.
